1. How do I make my finance payment and am I going to be charged a fee for paying it online?
A. You can make your finance payment either by sending in a check or giving us your credit card information. You may submit your credit card information online through our website with no additional fee; Checks can be mailed to our P.O. Box or street address.

3. How do I renew my policy?
A. In order to renew your policy, we need your completed and signed application along with the full premium. You may fax, email, or mail your applications in to us.
B. We currently underwrite and issue accident medical coverage for QBE Insurance Corporation, Certain Underwriters at Lloyd’s of London, American Sentinel Insurance Company, Aegis Security Insurance Company, Capital Indemnity Insurance Company, Platte River Insurance Company, Capitol Specialty Insurance Company, Commercial Travelers Mutual Insurance Company and Security Mutual Insurance Company. For liability insurance, we underwrite and issue coverage on the behalf of Capital Specialty Insurance Company and Capital Indemnity Insurance Company.

4. Why do I need to fill out new applications to renew if my numbers are the same as last year?
A. The policies we issue are non-renewing policies, therefore, the carriers require that we have updated information on file for every year that coverage is provided.

5. What is the agency info at the bottom of my application?
A. The agency information at the bottom of your application is only utilized if you go through an agency. If you are coming to us directly, this does not need to be filled out.

7. I need to add an additional insured to my policy, how do I get this done?
A. In order to add an additional insured to your policy, we need your request in writing. You may send this to us via e-mail, fax or mail. Make sure to include the name of the Policyholder, the certificate number, and where we can send the additional insured to.

8. How can I change my dates, limits, name, address or add participants to my existing policy?
A. In order to make changes to your policy, we need your request in writing. You may send this to us via e-mail, fax or mail. Please make sure to include the name of the Policyholder, your policy number, certificate number, and specific details as to what needs to be changed on your policy.

9. How long before I receive my policy?
A. Coverage is bound once we receive your completed application and full payment. Proof of coverage can be rushed if requested, and hard copies will be mailed upon completion shortly thereafter.

10. Do you provide business in all states?
A. Yes, we do provide business in all states; however, we are unable to provide a policy outside of the 50 states.

11. What is a minimum premium?
A. A minimum premium is the minimum amount that is required in order to bind coverage; if your rate calculations fall below the applicable minimum premium.

12. Are tournaments covered under the Martial Arts and Dance Studio policies?
A. Yes, tournaments are covered under the Martial Arts and Dance Studio policies if the participants of the tournament are the same as the participants stated on the enrollment form. If the tournament is for outside participants (anyone who wants to participate), then there is no accident/medical coverage in place. In order to cover outside participants, a special events accident policy for the tournament may be purchased. Liability coverage remains in place.

13. Can I purchase the liability coverage without the accident coverage?
A. No, an accident medical policy of at least $10,000 is required in order to purchase a participant liability policy. No accident policy is required in order to purchase a spectator liability policy.

14. Where can I send in my claims to?
A. Accident claims can be submitted to the Loomis Company. See the claim form for details. Liability claims can be submitted directly to our office via fax or email. Please make sure to include contact information, Policyholder name, certificate number, and any other information needed to submit your claim.

15. How can I check the status of my accident claim?
A. Please call the Loomis Company at 888-585-7065, as we do not handle our accident claims in office.

16. How do I cancel my policy? Will I be refunded?
A. You may cancel your policy by sending a request in writing via e-mail, fax or mail. Please be sure to include the name of Policyholder and certificate number. NOTE: Minimum premiums are fully earned at inception of the policy. Anything above the stated minimum premium may be pro-rated for a refund.

17. Are we covered if we travel to another state or outside the U.S.?
A. Yes, your coverage will follow wherever you go. However, claims or lawsuits must be brought into the U.S. in order to be submitted.

18. Can I get a sample policy?
A. Yes, we can provide a sample policy. When submitting your request, please provide the state you are in and the type of activity your policy will be covering.

19. Can you provide Worker’s Comp?
A. No, we do not provide Worker's Compensation
B. If this is a medical claim with the policy number beginning with 2003, 2004 or 2005 you will need to contact Commercial Travelers Insurance Company at 800-422-6200.

21. How can I get a quote?
A. The online applications are provided with the rates, and therefore are self quoting. You will know exactly what your premium is once you calculate your applications. Certain risks, such as events or facilities, are required to be looked at by an underwriter before a formal quotation may be provided. One of our representatives will be able to provide you with the appropriate quotation request, should this be needed.

22. What If I need my policy to run longer than what's stated in the brochure?
A. All of our brochures have the capability of being prorated to annual policies. Please make sure you send in your pro-rations when you submit your application.
To convert a policy to an annual policy:
1. Divide the per participant rate by the number of months the rate is set for
2. Multiply that number by 12
3. Use this rate for your participant rate when calculating your premium to reflect an annual policy term.

23. What do I do if I don't know how many participants I'm going to have yet but still need coverage bound?
A. We will issue your policy at minimum premium and you may adjust the policy to add more participants at a later date.

A. No, policies require 100% participation.
